Two historical campaigns illustrate how these dynamics manifest.
Loose Definitions and Repetitive Execution
When planning, a loose definition of success forces teams into a cycle of repetitive, high-risk execution.
Before WWII, promoters promised "pickle barrel" pinpoint bombing. In combat, the average circular error was 1,200 feet. A heavy bomber flying at 20,000 feet had only a 1.2% probability of hitting a 100-foot-square target.
Because a standard 500-pound bomb had a lethal radius of only 60 to 90 feet, planners had to deploy an average of 220 bombers to achieve a 90% probability of destroying a single target. This forced aircrews to fly repetitive, high-risk missions over heavily defended targets. The resulting compound risk drove loss rates to 28%–30% on critical missions, decimating crew morale and resulting in 58,000 casualties.
Deceptive Jargon and Self-Scoring Systems
Organizations often rely on automated systems that score success based on internal milestones rather than actual customer outcomes.
During the 1991 Gulf War, leadership praised the Patriot missile system's successful intercepts, reporting success rates of 70% to 100%. However, the technical definition of an intercept did not mean target destruction. In military jargon, it merely meant that the Patriot and target crossed paths in the sky.
Because target Scud missiles naturally fractured upon reentry, the Patriot’s radar locked onto the larger, highly reflective falling rocket debris. Proximity fuzes detonated near this metal, causing the computer to log a successful intercept while the live warhead descended untouched.
The Downstream Vulnerabilities of Misaligned Reporting
Misaligned reporting creates three main vulnerabilities:
1. Resource Exhaustion
Because the system could not discriminate between warheads and debris, crews fired three to six interceptors per Scud. This threatened to deplete critical theater stockpiles, which was especially hazardous given that only three PAC-2 missiles were completed when the crisis began.
2. Technical Drift
The Patriot's software accumulated a timing error of 1 microsecond for every second of operation. Left running continuously for over 100 hours, the accumulated one-third second drift caused the radar's search gate to look 700 meters away from the actual target, rendering the system blind to an incoming missile. This unmonitored drift directly resulted in 28 fatalities and 97 injuries at a Dhahran barracks.
3. Stakeholder Panic
Because the public assumed intercept meant absolute protection, citizens let their guard down. The subsequent realization of vulnerability caused public panic, resulting in 544 hospitalizations for anxiety, 230 self-inflicted injuries from unnecessary atropine injections, and 11 indirect civilian deaths in Israel.
Strategic Prescriptions for Leadership
To prevent these alignment risks in your own operations:
- Establish linguistic transparency: Do not allow specialized internal technical jargon to masquerade as an external success metric.
- Avoid self-scoring telemetry: Validate system predictions using independent, post-hoc physical outcomes.
- Monitor process drift: Implement controls to audit and reset compounding, micro-level errors in long-running processes.
